This is an authorization only. Billing will occur when the screening unit is shipped.
The screening cost is $200.00.
Shipping charges range from $15 - $65, based on your geographical location. The unit is delivered to you by UPS Second Day Air, if you reside outside the state of California. California shipments are by UPS Ground. A UPS return label is supplied in the package along with instructions. You must follow the return shipping instructions and screening time lines to ensure accurate recovery of your data.
Based on your mailing address, we allow seven days from our shipping date for you to complete the screening and return the unit to us. You may be assessed a $15/day charge after the seven day period, until Sleep Apnea Screening Services receives the returned equipment. If the equipment is not returned within 30 days, the assumption will be that it will not be returned. At that time, an additional charge of $760 will be charged to your on-file credit card. If the equipment is returned after the 30-day period, a refund of the balance will be reimbursed after all obligations have been met.
Additional charges will apply for multi-person screening (up to 3 screenings). A charge of $35 will be assessed for each additional screening report.
Sleep Apnea Screening Services will contact you within 24 to 48 hours, upon receipt of your order, for scheduling.
